This condition arises from irritation of the iliotibial band, a robust fascial structure extending from the hip’s tensor fasciae latae and gluteal muscles to the tibia. While often attributed to tightness, contemporary understanding emphasizes the critical role of muscular imbalances and biomechanical deficiencies, particularly in the hip musculature, as primary etiologies. The pathogenesis of ITBS frequently involves insufficient strength in the hip abductors and external rotators. Weakness in muscles such as the gluteus medius and minimus leads to compensatory movement patterns, increasing tensile stress and friction on the IT band as it traverses the lateral femoral epicondyle. This mechanical irritation precipitates inflammation and pain, characteristic of the syndrome. Therefore, effective management and prevention of ITBS hinge upon a targeted strength training regimen. This approach aims to restore optimal hip and knee mechanics by fortifying the musculature responsible for pelvic stability and lower limb alignment. Prioritizing strength over exclusive focus on stretching or foam rolling is crucial for long-term resolution. Key exercises for addressing these deficits include side leg raises, which directly engage the gluteus medius. Performed lying laterally, the movement involves abducting the top leg with controlled elevation and descent, emphasizing isolation of the hip abductors. This exercise promotes strength in the frontal plane, essential for stabilizing the pelvis during locomotion. Clamshells are another foundational exercise, targeting the gluteus medius and deep hip external rotators.
